Annual Self Assessment - 2026
Please submit one Self‑Assessment form per organisation, regardless of how many FEAST activities you have delivered during the year (for example, across multiple sites or age groups). This only needs to be completed once a year and will be requested ahead of our annual visit to your provision.

Self Assessment

The following questions ask you to assess your FEAST activities. These questions mirror those used by the FEAST Team during Provider Visits. Your responses help us understand where you are on your journey towards being the strongest FEAST provider possible, as well as identify where we may be able to offer additional support. You can find a copy of all the rating descriptions in the Provider Resources folder, under “Provider Visits.”

The Department for Education (DfE) requires that children funded through FEAST have access to the same overall food offer as children who pay to attend. If non FEAST-funded children are able to bring a packed lunch, you must ensure that FEAST‑funded participants also have access to a packed lunch style option that is prepared and paid for by you. This helps to ensure that FEAST‑funded participants are not visibly eating different food. We recognise that, for some providers, offering food as standard to non‑funded children is not feasible. This is acceptable, provided that appropriate measures are in place to remove stigma and, wherever possible, any visible differences between the food experiences of funded and non‑funded participants. For example, where paying children bring their own packed lunches, you may provide packed lunch style food for FEAST‑funded children. Under no circumstances should the food provided to FEAST‑funded participants be of a lesser quantity or quality than that offered to non FEAST‑funded participants. If you are one of the very few providers who do not offer paid for places, please confirm this here.

Where 1 is Highly dissatisfied and 5 is Highly satisfied. It is your responsibility alone to market your activities and we cannot amplify all marketing attempts. We also need to balance these fairly across the county and activity type. Finally, we will not amplify marketing that uses incorrect (or no) logos etc.
