Transport Request Form

Please fill in this form to request transport. Please note, you must have already booked your activities on HolidayActivities before requesting transport. Filling in this form does not automatically guarantee you will receive transport - we have a limited budget, but will try to accommodate as many requests as possible. Therefore you should only request transport if aboslutely neccessary - e.g. if your child has SEND and cannot attend any local FEAST activities or you live in a particularly rural area and do not have access to any personal or public transport. Please note, journeys are limited to 10 miles each way - unless there are exceptional circumstances (e.g. we understand that there are fewer specialist SEND activities, and the nearest one may be over 10 miles away). The deadline to complete this form is Wednesday 19th July however, we may close the form early if there is unprecedented demand. Please allow at least 4 days before following up about a transport request, we will need at least 4 working days to process the request and make the booking. The vast majority of children and young people eligible for FEAST live in North Yorkshire, and therefore one of the districts above. Under very specific circumstances, a child/young person from outside of North Yorkshire might have been given a FEAST code. In this scenario, and no other, choose Other.

We are only able to approve transport requests to FEAST activities relatively close to your starting/return address. When approving requests, the proximity to the activity, as well as the rurality of both addresses will be considered. We also appreciate that specialist SEND provision may incur a longer journey time.
